Dr. John Daniel SmithI am a Veteran.

of Conway, AR

October 11, 1947 - March 26, 2024

With profound sorrow and broken hearts, we announce the passing of our beloved Dr. John Daniel Smith. Dr. John departed this world peacefully on March 26, 2024 surrounded by his loved ones in Conway, Arkansas. Born October 11, 1947 in Camden, Arkansas, his life was a story of his dedication to God, his family and his patients. Called to medicine at the age of 10, he made it his mission to heal others with his God-given gift. For over fifty years, his dedication to medicine spanned his practice at the Banister-Lieblong Clinic, Chief of Staff for Conway Regional Medical Center and Team Physician for Conway High School (10 years) and University of Central Arkansas (27 years).

His life was anchored in his family. He shared a deep and abiding love with his wife, partner and soulmate, Carla Smith, from 1972 until his passing in 2024. Their marriage brought forth two remarkable children, Jacob Smith (Libby Smith) and Jessica Phaup (Doug Phaup). In his later years, he relished his role as Pops, showering love, attention and time with his five grandchildren, Stella Smith, Spencer Smith, Greer Smith, Oliver Phaup and Finley Phaup, who he deeply loved. He also cherished his relationship with his brother and best friend, David Smith (Shirley Smith), as well as his seven nieces and nephews.

He was passionate about the game of golf, had many putters and considered his trip to Scotland in 2011 with his brother, son, nephews and friends among one of the highlights of his life. He also prided himself on solving the daily crossword in record time (with his left hand). He always looked forward to watching a great movie or television series at home alongside his wife. Whether in the clinic, on the golf course, on the sidelines, or just hanging out, being the D.O.C, he never failed to leave those he encountered in better shape than he found them.

His legacy will continue to live in the hearts and minds of those fortunate enough to have known him. His compassion, brilliance and devotion to God, his family and his patients will be cherished forever. He will be remembered as an exceptional physician focused on the quality, not quantity, of his patients' care, routinely committed to going above and beyond whatever was necessary to treat his patients.

We would like to send a special thank you to Dr. Tim Freyaldenhoven for providing excellence in his recent attention, expertise and overall care he provided in the treatment of our dad/husband. Additionally, we would like to give thanks to Dr. Thad Hardin and Dr. Lander Smith for their unwavering support over the past weeks. Among the other special people in his life, the family would like to recognize Rene' Ford, the past and present doctors and staff of the Banister-Lieblong Clinic and the many patients' lives he touched over his entire career.

The family would like to extend an invitation to the public to attend a Celebration of Life service at Antioch Baptist Church on April 13, 2024 at 2:00p.m. to honor and celebrate his memory. Consistent with his wishes and in lieu of flowers, the family encourages donations to Antioch Baptist Outreach Ministry, a ministry that holds a special place in his heart.

Rest in peace, Dr. John. Your luminous spirit continues to inspire all who had the honor of knowing you. Your light shines on in the lives of those you touched, as a beacon of love, compassion and kindness. You will forever be remembered as a husband, father, grandfather, brother, uncle, healer, teacher, mentor and child of God. We love you!

My wife and I were blessed to know Dr. John for over twenty years, and even attended the same church for the last ten or so. He was such a fine physician and, even better, a man of God. He was the consummate professional and the best care giver in the world. Ironic how he was "Doctor John" all his life, never retiring. Apparently heaven needed a doctor, so why not grab the best and most godly one available We saw him together at the clinic not long ago and he was as sharp and witty as ever. Condolences to the family. Conway, and Arkansas, did not just lose a great man; the world lost a great man. We loved him and will continue to pray for the family, especially his brother David - Big Papa!
I must add that it was awesome seeing him rock out and sing along to the music at church. After all, he was a rock star in so many ways. God bless you all and know that his life touched multitudes, all of whom are blessed to have been a part of his.

John Smith family -- I was shocked to see John D's obituary. I was saved under Pastor John Smith at Calvary Baptist Church in Morrilton, AR. John D was my doctor with my first child and David Smith was my pharmacist while i was living in Conway. I have very fond loving memories of the whole family. Praying for you during this time of loss and praising the Lord knowing I will see them again.

Carla, Jessica, Jacob and David,
I was heartbroken to hear of Dr. John’s passing. He took great care of our entire family for many years. I will always be grateful for that but will be eternally grateful for the special care he gave to our precious mother. She loved and trusted John her entire life. When she couldn’t come to him, he came to her. Keeping you all in my prayers and sending love and hugs to each of you.

A piece of my heart has gone with my second dad. Dr. John has been my doctor for over 45 years and earned the name of Papa John. He was so compassionate that when he diagnosed my lung condition tears came to his eyes. I remember telling Papa John God and I got this. And we did. As years pass Papa John asked about my lungs. I told him, I didn't have a doctor for that condition any longer. I thought that doctor was going to tear me a new one. I could only Apologize. Believe me, I had an appointment that following week. Papa John did not play. He said what he meant and meant what he said. After getting married, he became my husband doctor and after having children he became my children doctor. My youngest son asked me who would he go to now. We will dearly miss Dr John. I you Papa John.

Carla & Family:

I'm very sad to learn of John's passing. I don't have to tell you how much he has meant to our family over the years. I will never forget when we lived down the street from your family back in the late 1970's, early 80's. The day my dad had his heart attack at home, I sent a neighbor kid on his bicycle to your house to get Dr. John. Within minutes, he pulled up and ran in the house to help and do what he could. That's the kind of guy he was. Of course he would help as a doctor, but the connection was deeper than that. We were close to your family. Over the years, John would always grin when I'd see him and call him "Trapper John, M.D." That show was popular back then, and the nickname just seemed to fit. Your family will always be special to me. Please know that I am here for you all if you need anything. My deepest sympathy, prayers, and love to all. Rest easy Trapper John, you will certainly be missed. Maybe you and dad can play a round of golf on that Heavenly golf course. Romans 8:28

Carla, Jacob, Jessica and family.
while saddened by the loss of Dr. John, he leaves behind so many memories by those he helped throughout his life.
We remember the pick up basketball games as a young man as well as taking care of our children. A very special memory of him coming to our home after the loss of our daughter. He was not on a call but as soon as he learned of her passing he came to help with questions we had but mostly to show us how much he cared about our family.
As so many others have expressed, we pray for you all and are thankful to have had him as our “family” doctor!

My sincere sympathy to the family, friends, staff and many patients of this awesome physician!!

He was not my physician but my precious sister in law, Janis Robbins. He has been her doctor since she was 19 or 20 years old, about 40 years!

She had an appointment with Dr. Smith about 3 or 4 weeks ago.
We had a conversation that day about her visit and Dr. John, she told me how long he had been her doctor, how he took care of her needs that day then stopped and ask her "what is going on with you today" He took his time for her care and also wanted to just talk and see what is going on in her life life that day!
I find this to be a true physician with a heart for his patients who took his oath serious!

How many times some of us have had doctor appointments you may sit for quite awhile waiting then ushered in and out so quickly!
Then when you get to the car you remember you forgot to ask about something else that you needed to to know what the doctor thought about it!
Not so for this amazing physician I have heard and read about over the past few days!

Dr. Smith will be sorely missed by so many, but for some reason God called this great man of God home! He is in Heaven with so many celebrating in a great reunion! Jesus has made a way that we all can see him and all our loved ones in Glory one joyous day!
Thank you Dr. Smith for taking such good care of my sister, Janis Robbins she loves you and at this time her sweet heart is broken!
But she knows she will see him again!
May God truly bless this family with His presence around them today and in the days ahead!
You have such wonderful memories share them often with each other!
And may you know the prayers for you all are flooding the sky on their way to the Throne of Grace!
God bless, my prayers are with you for all this Great Physician Dr. John Smith has left his mark on this community well!

Prayers for you all,
Deb Garrett

His love for God, his family and his patients was on-going. He touched so many lives and made such a difference in those he cared for. Steve and I felt like John was a part of our family, because he treated us as such. He will be missed by a multitude of people. His love for God and his family was beyond belief. God truly knew what he was doing when he instilled in John the desire to be a physician. That was so his calling and devine gift. Steve and I loved John.

My deepest sympathies to Dr. John’s family! Any of us fortunate enough to have known him is aware that the community is made lesser with his passing. I was referred to him by a friend when I first moved to Conway and from the first visit to his office, I knew he was different in the best way. His straight shooter way of giving you news was always appreciated and his kindness was always felt, even in the difficult talks. One of my biggest worries about moving away was that I’d never have another doctor like him and I was right, they don’t make them like that anymore. I hope the golf course in heaven had his tees ready and waiting because he deserves the best for giving his to the community for so long.

You deeply cared for me, my family, all these years. When I couldn't have our baby by myself, you came on your lunch hour, and delivered him by c-section. I will never forget our office visits, your wise directions for me, your words of kindness, wisdom, and love. As you did for all your life long patients who we will all miss you very much. I asked Dr. John one day not too long ago, when was he going to retire, he said, I love my job, I love my patients, why would I want to retire I was not looking forward to finding a new doctor in your retirement, and especially now, I am not excited about finding one. You cared so deep for so many of us. I will never have another doctor like you, you cannot be replaced, that must be why your patients never left. We are lifting your wife, your children, and your grandchildren in our prayers - who you bragged on so often during our visits. Prayers for your brother, and all your coworkers and patients. God had plans for you to leave this earth, and we will all get to see you again. Rest in Peace, my friend, my Doc. You will never by forgotten.

Dr. John Smith was my personal physician for over 40 years. I always felt that I was in the best hand with him. He never failed me. Now I will have to find a new primary care doctor, but I will never be able to replace that long held relationship that I had with MY doctor., John Smith.
My most heartfelt condolences to his entire family.

Dear Dr. John’s Family:
We’re so very sorry to hear this. Y’all are in our thoughts and prayers. We really appreciate his doctoring for me and my mom. He was a wonderful doctor and person but most importantly a friend. My mom Kathryn Smith worked with him for 6 years. We both loved him dearly and going to miss him terribly. We both have been heartbroken ever since we found out his passing.
With Love and Prayers for all the family.

Always so kind. Was so happy to run into him in the airport just a couple of months ago... he was off to see the grands with Mrs Carla. My most vivid memory is his red convertible in the 80s. I thought there was no one cooler driving that car. Can still picture him parking it at church! He left a legacy of compassion and care in Conway and will be missed.
